The Incision Point

The healthcare earth is shaking underneath us. Most people aren’t paying attention. I am.

Welcome to the front lines of the arthroplasty revolution. I am Dr. Michael Meneghini, and I’ve spent the last 20 years unapologetically obsessed with one goal: delivering lower-cost, higher-quality orthopedic care while fiercely protecting the doctor-patient relationship.

This Substack is for the surgeons, industry executives, and healthcare leaders who realize that the status quo is unsustainable. Whether it’s the “administrative creep” of hospital bureaucracy or the shifting economics of Ambulatory Surgery Centers (ASCs), the industry is changing. You can either be a passenger or the driver.

Who is Michael Meneghini, MD?

I approach medicine with the mind of an engineer, the hands of a surgeon, and the grit of an entrepreneur.

  • The Surgeon: I have performed over 10,000 hip and knee replacements. My clinical focus is on preeminent outcomes, specializing in same-day discharge and rapid recovery protocols that get patients walking in as little as 55 minutes post-op.

  • The Engineer: My background in engineering allows me to serve as a principal designer for the world’s leading implant systems—from the Triathlon Cementless TKA to the EMPOWR hip and knee systems.

  • The Entrepreneur: I am the Founder and CEO of the Indiana Orthopedic Institute (IOI). In just 36 months, we scaled from a single surgeon to a powerhouse of 13 surgeons across three markets, proving that independent, vertically integrated practice models aren’t just viable—they are the future.

  • The Leader: I am currently serving as the 2025 President of the American Association of Hip and Knee Surgeons (AAHKS) and as a Chief Executive at HOPCo, a $1.6B valuation orthopedic management company.

Subscribers receive high-signal deep dives into the three pillars of modern orthopedics:

  1. The Economics of Arthroplasty: Brutally honest takes on the transition to ASCs, the “money side” of rapid recovery, and how surgeons can reclaim their autonomy from hospital administrators.

  2. Clinical Innovation & Data: We go beyond the abstract. Expect rigorous analysis of my 225+ peer-reviewed publications, covering everything from Extended Oral Antibiotic Prophylaxis (EOAP) to the clinical realities of Modular Dual-Mobility constructs.

  3. The “Straight Talk” on Leadership: I believe in “Grit”—passion and perseverance. I share the strategies I use to manage a world-class surgical volume, multiple industry partnerships, and a family of six, all while needing only five hours of sleep.
